Nice list, but let me add something. As for item 5 - it is more preferable to use hyphens (-), as soon as Google understands them as separators correctly.

FWIW, my own file layout was sort of a bad remnant left over from how I migrated my site files around when I made my comic just part of my site rather than the main thing (as well as how it used to just be a single series). Today I finally got around to fixing that. It's something I'd been planning on for a while but this article finally provoked me into doing it.

Just my personal experience, on my own site. I've noticed images load first when located in shallower directories, as opposed to those set deeply into something. Of course it's going to depend on the size of the image and location in the page as well, and probably on other factors I hadn't considered. Regardless, really I'm just saying people should "organize their shit, yo" but not to over-organize/Monk it. :D

I recently moved almost all my root-folder images into images folders for both my webcomics. I'd been in the habit of dumping certain images into the root folders, and while it was a bit convenient it was also a bit messy. (Especially when I had all my skin images in the root folder...ew)

My FTP and website folders aren't all exactly the same, but only in places where organization is a lot more important on the HD. Although over time it's making less and less sense to me why I did certain things in the first place...XD

My PC's folders in general are faaar more unorganized, even with all those tons of folders. %D And there's no easy way to redo them.

Do you have any actual evidence that deep pathing causes an actual measurable performance hit on sites which aren't handling thousands of requests per second? Considering how much stuff a webserver has to do, and how much filesystems are optimized for actually getting stuff out of directories, I don't think the whole /images/characters/mammals/adult/sparkles/this_morning/breakfast.jpg thing is actually going to be a concern, aside from it making for ridiculously bloated URLs.
